Thank you for your interest in applying for a PADS Mobility Service Dog. Completing this application form is the first step in our process. Once we receive your submission, our Applicant Services Team will review it according to the schedule posted on our website. You will then be contacted by email regarding whether your application will proceed to the interview stage.

Please note: submitting an application does not guarantee that a Service Dog will be placed with you. Placement depends on eligibility, suitability, and availability of an appropriate match.

Our Application Process

1. Application Form – Submit this form with accurate and complete information.
   
   
2. Interview – Conducted online (MS Teams) or in person.
   
   
3. Supporting Documents & Payment of Application Fee – Completion of a Medical History Form, submission of two reference letters, and payment of a $50 non-refundable application fee.
   
   
4. Full Application Review – Confirmation that all required documents have been received. At this stage, you are considered for a PADS Mobility Service Dog.
   
   
5. Waitlist & Updates – PADS will contact you when a potential match is available and will follow up periodically regarding any changes in your situation.
   
   
6. Team Training – Once a match is confirmed, you will be given information about next steps, including a $250 Team Training Fee and arrangements for your training program.

Care & Training Responsibilities

• Daily exercise and enrichment appropriate to the dog’s needs
• Ongoing reinforcement and maintenance of training (with and without PADS support)
• Regular grooming and health care routines

Financial Responsibilities

• Day-to-day costs: food, toys, grooming supplies, and other essentials
• Veterinary care: routine check-ups, vaccinations, urgent medical needs
• Application Fee: $50 (non-refundable, payable after a successful interview)
• Team Training Fee: $250 (payable before training begins, once matched)

Following completion of this application form, we will review this (in line with the schedule on our website) and inform you whether you will move onto the interview stage, or whether we have decided that we are not moving forward with your application.
